From the SEIB website: 

"As a SEHIP member, you can assess your current health condition by participating in the annual Wellness Screening. During the screening, the SEIB will be checking for certain risk factors, such as elevated blood pressure, blood sugar, cholesterol, or BMI. Individuals with one or more chronic conditions are more likely to suffer from the effects of infectious disease than those with few or no health conditions. If certain risk factors are identified, you will be referred to your health care provider for a follow-up visit. Once the wellness screening and office referral (if applicable) are complete, you will receive a $25 monthly wellness premium discount."
